Be a part of the change your community wants! Take the MA Department of Public Health Community Health Equity Survey in our Community Room and receive a visa gift card.
The Community Health Equity Survey uplifts marginalized communities that are often underrepresented in data pertaining to their health and well-being. By taking the survey, you are adding your experience and expertise, and ensuring that your community is represented.
The first 58 people, ages 12 and up, to visit the Transhealth Community Room and complete the survey in-person will receive a $30 visa gift card. Limit one per person. Youth 12-17 must be accompanied by a parent or guardian to receive the gift card.
